CORE Vascular Services is a provider for most of North Dallas’ major health insurance plans.
Prior to your appointment in our office, it’s a good idea to check if care by our physicians is covered by your health plan and to learn about your plan’s covered services, payment terms and referral policy.
Regardless of your insurance coverage, you are responsible for payment for the services performed. Our staff will be happy to submit initial claim forms to your insurance carrier. If you are unable to show proof of coverage, we expect payment for services at the time of your office visit.
Billing requirements
Patients must pay any co-payments at the time of their visit. For your convenience, our office accepts MasterCard, Visa, Discover, Care credit, debit cards, personal checks and cash as payment for services.
Additionally, we have a cash based program for all in-office visits, test and procedures for patients with no insurance or deductibles too high to use their insurance.
